Proper Use:
- Dose: 300–600mg standardized extract daily (withanolides 5–10%)
- Duration: Clinical effects visible after 4–8 weeks
- Timing: Take with food — reduced GI discomfort
- Evidence-backed: chronic stress, cortisol, sleep, athletic recovery, mild anxiety
Interactions You Need to Know:
- + sedatives / benzodiazepines / alcohol → additive CNS depression, enhanced sedation
- + levothyroxine → may increase T3/T4 — risk of thyrotoxicity
- + immunosuppressants → stimulates immune response, direct conflict + antidiabetics → additive hypoglycemic effect
- + antihypertensives → mild hypotensive effect may potentiate medication
CYP450 — Polypharmacy Risk:
CYP3A4 inhibition → ↑ statins, benzodiazepines, cyclosporine, tacrolimus ▸ CYP2D6 inhibition → ↑ beta-blockers, tricyclics, codeine ▸ CYP2C9 possible inhibition → ↑ warfarin, ibuprofen, losartan
→ Warfarin + ashwagandha = elevated INR risk
→ Tacrolimus/cyclosporine = narrow therapeutic index + inhibition = serious risk
→ CYP2D6 slow metabolizers — effects more pronounced
Supplement history is just as important as medication history.
Absolute Contraindications:
- Pregnancy — uterotonic activity, miscarriage risk
- Autoimmune conditions (MS, lupus, RA) — may worsen disease
- Hormone-sensitive tumors — modulates DHEA and testosterone
- Active thyroid disease — lab monitoring required first
Special Caution:
- Polypharmacy (5+ medications) — verify before introducing
- Perioperative — discontinue 2 weeks before surgery
- Children and adolescents — insufficient clinical data
