Eric Lameignere: Are We Measuring the Right Sperm DNA Fragmentation?
Eric Lameignere, Co-Founder of MovaLife Microrobotics, shared a post on LinkedIn about a paper by Tianqin Den et al. published in Translational Andrology and Urology:
”Are we measuring the right sperm DNA fragmentation?
Sperm DNA Fragmentation Index (SDF) has become a key biomarker in male fertility assessment. However, a new study published in Translational Andrology and Urology suggests that we may have been looking at the wrong metric all along.
Traditionally, SDF is measured across the entire sperm population, including both viable and non-viable spermatozoa. The problem?
Only viable sperm can fertilize an oocyte and contribute to embryo development.
The researchers analyzed 227 men undergoing fertility evaluation and compared conventional total SDF with viable SDF, a measurement focused exclusively on living, fertilization-competent sperm.
The results were striking:
- Viable SDF showed stronger associations with sperm quality than total SDF.
- A new clinical cutoff of 9.79% viable SDF demonstrated good diagnostic performance (AUC 0.813, sensitivity 76%, specificity 66%).
- Significant discrepancies were identified between total and viable SDF classifications:
23 men had high total SDF (>25%) but low viable SDF (<9.79%)
44 men had normal total SDF (<25%) but elevated viable SDF (>9.79%)
Conventional SDF may both overestimate and underestimate clinically relevant DNA damage, depending on the patient.
DNA integrity should be assessed within the functional sperm population, not across the entire ejaculate. This concept of a ‘functional DNA damage threshold’ could improve patient stratification and support more personalized fertility treatment decisions.”
