Houssein El Hajj: Is the 2 cm Cutoff Enough for Fertility-Sparing Surgery in Early Cervical Cancer?
Houssein El Hajj, Gynecologic Oncologist at Gustave Roussy and President of the European Network of Young Gynae Oncologists, shared a post on LinkedIn about a paper he co-authored with colleagues published in the International Journal of Gynecological Cancer։
“Is a 2 cm cutoff really precise enough when considering fertility-sparing surgery for early cervical cancer?
Pleased to share our new commentary in the International Journal of Gynecological Cancer: ‘Is conization with lymph node dissection safe for FIGO 2018 stage IB1 cervical cancer measuring 1 to 2 cm?’
Prospective data from ConCerv and GOG-278 have strengthened the case for surgical de-escalation in carefully selected patients. But an important question remains: should a sub-centimeter tumor and a tumor approaching 2 cm really be considered equivalent?
In this commentary, we discuss evidence suggesting that tumor size within the 1–2 cm range and lymphovascular space invasion may be important determinants of recurrence risk, while increasing surgical radicality itself may not necessarily improve oncologic outcomes.
The goal is not to move automatically toward more radical surgery, but rather toward better risk stratification and more individualized fertility-sparing decision-making.
More prospective data is needed to identify which patients with 10–20 mm tumors can safely undergo conization and lymph-node assessment, and whether some may benefit from closer surveillance.
