Luca Tramontano: Who May Truly Benefit from Fertility Preservation in Endometriosis?

Luca Tramontano, FMH Specialist in Gynecology and Obstetrics at WENGER, Endometriosis and Fertility Center, shared a post on LinkedIn about a paper he co-authored with colleagues published in Medicina:

”Who may truly benefit from fertility preservation in endometriosis – and when should we intervene?

In this review, we examine the latest evidence on oocyte vitrification, ovarian surgery, ovarian reserve, disease phenotype, stimulation strategies, reproductive outcomes, and the role of coexisting adenomyosis.

One of the key messages is simple but important:

Fertility preservation should not mean systematic oocyte freezing for every woman with endometriosis.

Current evidence instead supports early reproductive counselling and an individualised approach, with particular attention to women with bilateral or recurrent ovarian disease, previous or planned ovarian surgery, and delayed reproductive plans.

Another crucial point is the distinction between preserving oocytes and preserving fertility.

Oocyte banking can preserve an additional reproductive option, but it cannot eliminate the effects of ageing, disease progression, tubal or uterine factors – particularly in women with associated adenomyosis.

A field that is rapidly evolving, but where patient selection, timing and shared decision-making remain at the centre of good clinical practice.
