Mauro Francesco Pio Maiorano: Why Fertility Preservation Matters Before Gonadotoxic Treatment

Mauro Francesco Pio Maiorano, Gynecologist at the Local Health Authority of Barletta-Andria-Trani, shared a post on LinkedIn about a paper he co-authored with colleagues, published in Reproduction and Fertility:

“I’m pleased to share our newly published systematic review and meta-analysis:

‘Gonadotropin-releasing hormone agonists during cyclophosphamide for ovarian protection in systemic lupus erythematosus’, now published in Reproduction and Fertility, the official journal of the Society for Reproduction and Fertility.

Fertility preservation is an important but sometimes overlooked aspect of care for young women with systemic lupus erythematosus (SLE) receiving cyclophosphamide, a treatment associated with a substantial risk of premature ovarian insufficiency.

In our meta-analysis of 5 comparative cohort studies including 162 women, GnRH agonist co-treatment was associated with a marked reduction in the risk of premature ovarian insufficiency, and no evidence of increased serious adverse events.

These findings reinforce the importance of discussing reproductive health and fertility preservation before potentially gonadotoxic treatments, extending the conversation beyond oncology and into the management of autoimmune diseases.

As always, I would like to thank my co-authors Gennaro Cormio, Vera Loizzi, Brigida Anna Maiorano, Giacomo Corrado and Erica Silvestris for their collaboration.
