- One presentation that particularly stood out to me focused on endometrial preparation strategies for frozen embryo transfer (FET).
- A network meta-analysis comparing Hormone Replacement Therapy (HRT), HRT + GnRH agonist, ovarian stimulation cycles, true natural cycles, and modified natural cycles found that:
No endometrial preparation strategy demonstrated superiority for live birth rates.
No protocol showed better clinical pregnancy rates.
No significant differences were observed in miscarriage rates.
This reinforces an important clinical principle:
Protocol selection should be individualized—not standardized.
- When efficacy is comparable, the choice of protocol should be guided by:
- Menstrual cycle regularity
- Patient preference
- Monitoring burden
- Scheduling flexibility
- Safety considerations
- Clinic logistics
- Rather than expecting one protocol to consistently produce superior reproductive outcomes, we should focus on selecting the approach that best fits each patient’s clinical circumstances.
