Justin Houman: What Is the Male Fertility Workup, and What Does It Look Like?

Justin Houman, Men’s Health Urologist at Tower Urology at Cedars-Sinai Medical Center, shared on LinkedIn:

“A quick reference for primary care colleagues on what happens when you send a male patient for a fertility evaluation, since many men arrive not knowing what to expect.

The workup is four parts:

  1. a directed history (duration of attempted conception, prior paternity, medications),
  2. a semen analysis as the baseline (concentration, motility, morphology, occasionally signs of infection),
  3. labs covering the fertility hormones plus testosterone and estradiol, and
  4. a physical exam with scrotal ultrasound when indicated to evaluate for varicocele or other scrotal abnormalities.

It is noninvasive, and it produces a complete baseline from which nearly every male-factor question can be worked up or ruled out. Early evaluation of both partners saves couples months.
