Justin Houman: Sleeping 6 Hours? Your Testosterone Is Taking a Hit

“A note for primary care and sleep medicine colleagues on the low T workup.

Before testosterone replacement is on the table, sleep deserves a real look.

Nocturnal testosterone production is sleep-dependent: it rises from sleep onset, peaks around the first REM episode, and is blunted by fragmentation.

One week of 5-hour nights lowered daytime testosterone 10 to 15 percent in healthy young men (Leproult and Van Cauter, JAMA 2011).

Obstructive sleep apnea is the larger clinical issue. It roughly triples the risk of low testosterone; most cases are undiagnosed, and the presenting complaint – fatigue, non-restorative sleep, morning headaches – is routinely attributed to low T instead.

Treating the apnea improves testosterone in a meaningful share of these men, and the AUA guideline cautions that testosterone therapy can worsen untreated sleep apnea in some patients.

Order of operations matters: screen and treat the apnea first, then reassess whether TRT is still indicated.
