Khalil Faaed: Embryo Arrest – The Stage Matters, the Biology Tells the Story

“Embryo arrest is not one problem.
The stage of arrest may reveal the biological process that failed.
From fertilization to blastocyst formation, embryonic development is a sequence of highly coordinated biological transitions.
When development stops, the critical question is not simply: Why did the embryo arrest?
It is: At which developmental checkpoint did the embryo lose its capacity to continue?
- Day 1 (Zygote):
Fertilization, oocyte activation, and pronuclear development become important considerations.
- Day 2 (Early cleavage):
Cleavage dynamics, chromosome segregation, and gamete competence become increasingly relevant.
- Day 3 (4–8 cells):
The maternal-to-embryonic transition becomes critical, including embryonic genome activation and chromosomal integrity.
- Day 4 (Morula):
Compaction, cell adhesion, polarity, metabolism, and genomic competence become central to continued development.
- Day 5–6 (Blastocyst):
Successful blastulation requires coordinated cell differentiation, trophectoderm and inner-cell-mass development, metabolism, and appropriate cellular organization.
And throughout every stage, the laboratory environment remains part of the biological system.
- Temperature.
- pH.
- Oxygen.
- Osmolality.
- Culture media.
- Oil.
- Handling.
- Environmental contaminants.
Therefore, embryo arrest should not be interpreted as a single diagnosis. The timing of arrest is a biological clue. Perhaps the more important question in modern embryology is not:
Why did this embryo stop?
but:
What developmental checkpoint did this embryo fail and what does that failure tell us about embryo competence?
Understanding embryo arrest means moving beyond morphology alone and toward the biology of developmental competence.”
