Margaret Cali: The Quiet Load of IVF

Margaret Cali, IVF Readiness Integration Strategist, shared on LinkedIn:

“A woman can be medically prepared for IVF and still be carrying a great deal emotionally.

  • The blood tests may be complete.
  • The protocol may be ready.
  • The appointments may be booked.
  • The consent forms may be signed.

From a clinical perspective, treatment may be ready to begin.

But that does not necessarily mean the woman feels emotionally prepared for what treatment may ask of her.

She may still be carrying previous disappointment, fear about what comes next, the strain of waiting, the pressure to keep functioning, and what she is carrying from what has already happened.

This is not a failure of preparation.

It may simply mean that most of the preparation she has been given has focused on the medical side.

That distinction matters.

Emotional readiness is not about becoming calm, positive or fearless.

It is about making space to notice what is already there, what feels hardest, and what support may help her feel steadier as treatment begins.

I call this The Quiet Load of IVF.

  • The part that may not appear on the treatment plan.
  • The part that may not be visible in the consultation.
  • The part a woman may still be carrying while looking completely prepared on the outside.

One question I keep coming back to is:

‘What is taking more from her than anyone can see?’

For those working in fertility care:

Where in the treatment pathway have you found space for the part of preparation that does not appear on the medical checklist?”
