Research into novel biomarkers for Polycystic Ovary Syndrome (PCOS) and Diminished Ovarian Reserve (DOR) is opening new avenues for understanding reproductive health, diagnosis, and individualized fertility care.
PCOS: Beyond Conventional Markers
PCOS is highly heterogeneous, making diagnosis and risk assessment challenging.
Recent research is exploring combinations of:
- MicroRNAs
- Metabolomic Signatures
- Hormonal and Metabolic Biomarkers
DOR: Looking Beyond AMH
Emerging research is therefore examining the follicular microenvironment through:
- Proteomics
- Inflammatory markers
- Follicular-fluid biomarkers
- Metabolomics
- Molecular signatures associated with follicular function
A 2026 study using targeted proteomics identified a four-protein follicular-fluid signature involving ADA, VEGFA, 4E-BP1, and MMP-1 that showed promising diagnostic performance for DOR in its study cohort.
Where Could Biomarker Research Take Fertility Medicine?
The bigger question is not simply ‘Which biomarker is best?’
It is:
Can multiple biological signals be combined to create a more complete picture of an individual patient’s reproductive health?
Future research may explore:
- Earlier identification of reproductive risks
- Better characterization of PCOS phenotypes
- Improved assessment of ovarian reserve
- Molecular profiling of the follicular environment
- Prediction of treatment response
- More individualized fertility strategies
- Integration of biomarkers with AI and clinical data
