Karthik Raj: Asherman’s Syndrome – When the Endometrium Cannot Regenerate Normally
Karthik Raj, Clinical Embryologist at The Boon IVF and Fertility, shared on LinkedIn:
“Asherman’s Syndrome: When the Endometrium Cannot Regenerate Normally
Asherman’s syndrome is characterised by the formation of intrauterine adhesions (IUAs), often following trauma to the basal endometrial layer, particularly after pregnancy- related uterine instrumentation. Damage to the regenerative endometrium can result in fibrosis, cavity distortion and impaired endometrial function.
Clinically, the condition may manifest as hypomenorrhoea, amenorrhoea, infertility, recurrent pregnancy loss and adverse pregnancy outcomes. Beyond mechanical obstruction, disruption of endometrial architecture, vascularity and receptivity may compromise implantation and placentation.
Hysteroscopic adhesiolysis remains the principal treatment; however, recurrence is common in severe disease, and anatomical restoration does not necessarily translate into functional endometrial recovery. Persistent basal-layer damage presents a major challenge, with regenerative therapies remaining an evolving area of research.
The central challenge is not merely restoring uterine cavity anatomy, but recovering the endometrium’s capacity to regenerate, support implantation and sustain pregnancy.”
