“Implantation failure: when embryo quality is not the full story
Despite major advances in embryo selection and IVF technologies, implantation failure remains a key limiting factor in ART. Increasingly, evidence points to the endometrium itself, specifically its local immune environment, as a critical and underexplored determinant of success.
A recent comprehensive review highlights that uterine immunity is highly specialized, locally regulated, and fundamentally different from systemic immunity. Balanced immune activation during the implantation window is essential to support adhesion, controlled trophoblast invasion, angiogenesis, and immune tolerance. Dysregulation, whether excessive or insufficient, may compromise these processes and contribute to recurrent implantation failure or pregnancy loss.
Importantly, the review emphasizes that blood-based immune tests cannot reflect the uterine immune state, underscoring the biological rationale for endometrial immune profiling as an emerging approach to better understand uterine receptivity and to support personalized decision-making in selected patients. As reproductive medicine continues to move toward precision approaches, endometrial immune dysregulation is becoming an essential part of the scientific conversation around implantation biology.
