Alberto Sifuentes Giraldo: Innovative CAR–T Cell Therapy Promises SLE Remission and Fertility
Alberto Sifuentes Giraldo, Rheumatologist, Medical Doctor at National University of San Marcos, shared a post on LinkedIn:
“BCMA/CD19 dual-target CAR–T cell therapy can induce long-term systemic lupus erythematosus (SLE) remission, preserve fertility, and enable successful, healthy pregnancies/neonatal outcomes, as evidenced by the interesting case of a 24-year-old female patient with refractory class IV lupus nephritis who received this therapy and achieved sustained molecular remission (>30 months) with no flares:
- She had two spontaneous pregnancies (conceiving at 6 and 21 months post-infusion), resulting in term vaginal deliveries of healthy female infants.
- No CAR–T cell DNA was detected in maternal blood, breast milk, placenta, or infants’ blood (at birth and follow-up), confirming no vertical transmission.
- Both infants showed normal growth, neurodevelopment, and immune function during follow-up.
The patient’s quality of life markedly improved post-therapy.
This appears to represent one of the first detailed reports addressing fertility and pregnancy safety in the context of this emerging treatment for SLE, highlighting the need for pregnancy-related guidelines in post-CAR–T autoimmune patients.
