Sarah Lewen: Carousel Fertility Is Built to Help Clinics Win the Next Decade

“I have chosen a fertility clinic four times in seven years.

Each time, I did what every diligent patient does: I opened the success-rate tables. I sorted. I compared. I made a spreadsheet, because I am who I am. And I told myself I was being rigorous.

It took me until the fourth time to notice what I was actually looking at.

A success rate answers one question: of the patients who stayed, how many succeeded? It is a real number, honestly reported, and clinics work hard to earn it.

But it is silent on the question that defined my seven years, and defines the journey for most of the women I’ve interviewed: what happens to the patients in between – between cycles, between decisions, between the appointment and the next hard conversation. Who stays supported enough to keep going at all.

I spent ten years deeply examining retention at Amazon, Talkspace, and SonderMind, and here’s what I know from that seat: any industry measures what its system reports, and the fertility reporting system was built around outcomes per cycle – not continuity per patient. Clinics aren’t choosing the wrong number. They’re competing on the only number anyone hands them.

Which means the most important metric in fertility care doesn’t have a scoreboard yet: the return rate. How many patients come back for the care they still need – because the experience held them, not just the medicine.

The clinics that figure out how to win on that number will define the next decade of this industry. We’re building Carousel Fertility to be the layer that helps them do it.”
