Hilary Friedlander: Advocating for Comprehensive Fertility Care
Hilary Friedlander, Reproductive Endocrinologist at Atlantic Reproductive Medicine, shared a post on LinkedIn:
“There’s a lot of buzz lately about restorative reproductive medicine (RRM). And while the concept may sound innovative at first, the truth is this is what REI physicians have been doing all along.
I spent my morning commute listening to the Fertility and Sterility Roundtable podcast on Restorative Reproductive Medicine and it inspired me to add to this (very necessary) conversation. Episode here.
We already diagnose, restore, and optimize natural fertility. Comprehensive evaluation, hormonal optimization, ovulation support, timed interventions, and minimally invasive surgery are foundational to our specialty.
Where we diverge is when ‘restoring’ isn’t enough.
As REIs, we are fortunate to have advanced reproductive technologies, especially IVF, in our toolkit. These treatments are not shortcuts or ‘giving up too soon.’ They are medically necessary options for many patients including those with:
- Absent or severely damaged fallopian tubes
- Severe male-factor infertility, where ICSI is required for fertilization
- LGBTQ+ individuals and couples, for whom IVF is an essential and affirming pathway to family building
- Complex reproductive disorders that cannot be corrected medically or surgically
To suggest that IVF is optional, avoidable, or inferior, dismisses the lived realities of these patients and the decades of scientific progress spent refining IVF into the safe and effective treatment it is today.
Optimizing natural fertility is essential but limiting care to ‘restorative’ methods alone risks leaving many patients without the chance to build the families they dream of.
My goal is to offer patients the full spectrum of evidence-based care, grounded in science, compassion, and respect for the diversity of modern families.
I’m grateful to the leaders in our field who speak out against this RRM narrative and advocate for comprehensive, evidence-based fertility care that truly serves all patients.”
