Khalil Faaed: A Beautiful Embryo Does Not Guarantee a Competent Oocyte
Khalil Faaed, Intern of IVF/ICSI and Clinical Embryology at Indira IVF, shared a post on LinkedIn:
“A Beautiful Embryo Does Not Guarantee a Competent Oocyte: What SER Aggregates Reveal About Hidden Cytoplasmic Quality in IVF
Embryo morphology remains a cornerstone of embryo selection in IVF. However, morphology alone may not fully reflect the developmental competence of the originating oocyte.
Aggregates of Smooth Endoplasmic Reticulum (aSER) represent a well-recognized cytoplasmic dysmorphism characterized by abnormal clustering of calcium regulating organelles within the ooplasm. Given the essential role of the smooth endoplasmic reticulum in oocyte activation and early embryogenesis, these abnormalities may indicate underlying disturbances in cytoplasmic maturation.
Interestingly, oocytes exhibiting SER aggregates often achieve normal fertilization and can generate morphologically acceptable embryos. Nevertheless, several studies have reported reduced implantation potential, suggesting that conventional morphological assessment may overlook critical aspects of oocyte competence.
This highlights an important principle in reproductive medicine:
Embryo morphology reflects appearance, whereas developmental competence reflects biology.
As IVF moves toward more comprehensive approaches to embryo assessment, understanding the biological significance of cytoplasmic abnormalities such as SER aggregates may improve our ability to identify embryos with the highest reproductive potential.
The future of embryo selection may depend not only on evaluating the embryo itself, but also on understanding the quality of the oocyte from which it originated.”
