“Beyond Motility: Is Sperm DNA Integrity the New Gold Standard for Sperm Preparation in IVF?
Conventional sperm preparation has long been evaluated by improvements in sperm concentration, progressive motility, and morphology. While these parameters remain fundamental, they do not adequately reflect the biological and genetic competence of sperm. Increasing evidence suggests that sperm DNA integrity and oxidative stress are more closely associated with fertilization, embryo developmental potential, implantation, and ultimately reproductive outcomes.
Established techniques such as Swim-Up and Density Gradient Centrifugation (DGC) effectively isolate motile sperm; however, centrifugation may increase oxidative stress, potentially compromising DNA integrity in susceptible sperm populations. In contrast, microfluidic sperm selection eliminates centrifugation and more closely replicates physiological sperm migration, consistently enriching sperm with lower DNA fragmentation, reduced oxidative stress, and superior chromatin integrity.
This evolving evidence supports a fundamental shift in assisted reproductive technology from selecting sperm based primarily on motility to selecting sperm based on genetic and functional competence. As ICSI requires only a single sperm for successful fertilization, the biological quality of that sperm may be more critical than the total number recovered.
The future of sperm preparation may therefore be defined not by how many motile sperm are selected, but by how effectively a technique preserves and enriches sperm with intact DNA. Is sperm DNA integrity poised to become the new gold standard in IVF?”
