Mostafa Maghraby: Does Sperm DNA Fragmentation Really Matter in ICSI?

“Sperm DNA Fragmentation: Does It Really Matter in ICSI?

We often focus on sperm concentration, motility, and morphology when evaluating male fertility.

But there is another question that may be equally important:

What is the quality of the DNA carried by the sperm?

Sperm DNA Fragmentation (SDF) refers to damage or breaks within sperm DNA.

And this raises an important clinical question:

Can a sperm with good morphology still carry damaged DNA?

The answer is yes.

A sperm may look acceptable under the microscope while still having a higher level of DNA fragmentation.

Why does this matter in ART?

Sperm DNA integrity may influence:

  • Fertilization and early embryo development
  • Embryo developmental competence
  • Blastocyst development
  • Implantation potential
  • Reproductive outcomes in selected patients

But SDF should not be interpreted in isolation.

A high SDF result does not automatically mean that ICSI will fail.

Instead, it should be interpreted alongside:

  • Semen parameters
  • Clinical history
  • Previous ART outcomes
  • Laboratory findings
  • The individual patient’s reproductive context

This is where the role of the embryologist and andrologist becomes important:

The goal is not simply to find a sperm that moves.

The goal is to understand the quality of the sperm population and, when appropriate, consider strategies that may help select the most suitable sperm for ART.

Because under the microscope…

Morphology tells us what we see. DNA integrity may tell us what we cannot see.”
