Maternal and perinatal outcomes of pregnant women with echocardiographically high probability of pulmonary arterial hypertension

In this retrospective cohort of 62 pregnancies (60 women) at a quaternary referral center (2015–2022):

  • Cardiac complications occurred in 17.7%, mainly heart failure; maternal mortality was 6.5%.
  • Fetal growth restriction occurred in 27.7%; median gestational age at delivery was 37.1 weeks.
  • Exploratory analysis: a PASP threshold of 64 mmHg predicted cardiac complications (AUC 0.749), to be interpreted cautiously given sample size and scope/design.

Maternalmortality was lower than historically reported, but the burden of cardiac complications remained substantial—underscoring the need for multidisciplinary care and counseling in this high-risk population
