Sergei Gorlovetsky: 2 Days at the AIFertility World Conference Left Me with 1 Observation…

“Two days at the AIFertility World Conference in Cascais left me with one observation: the most persuasive numbers on screen were not about embryos.

Embryo selection is where AI in IVF began, and it is now being held to the standard it asked for. One session walked through a randomized trial of AI embryo selection: 46.5% with AI, 48.2% with standard morphology, non-inferiority not demonstrated. The speaker’s point was not that the work failed. It was that morphology is a cheap, immediate benchmark, and that time-lapse and PGT-A went through the same reckoning.

The operational talks told a different story.

  • Sama Fertility counted 177 manual touches in a single IVF cycle and showed a path to 11.
  • FertilAI showed a Manchester clinic leveling its retrieval days across the week with the same total workload.
  • Kai Health asked the question most of the room was circling: each AI tool solves one problem, so who connects the records, decisions, and handoffs around it?

Three things I took home:

  1. The bottleneck in a fertility clinic is the work between decisions, more than the decisions themselves.
  2.  Nobody serious proposed removing the clinician. Every credible design had a person approving and an agent carrying out, with rules checked before anything runs.
  3. Governance came up as a design requirement, from Brazil’s new CFM resolution to a regulatory session arguing that most AI submissions fail on documentation, not on the model.

This is the problem our team works on at Fertiligent AI, so I am not a neutral observer. But it was useful to see how many groups, on four continents, have arrived at the same place independently.
