“The most common pattern I still see in 2026.
She has done every test. Hormone panels. AMH. Antral follicle count. Thyroid. HSG. All of it. Over months. Sometimes over a year.
He hasn’t had a semen analysis done yet.
We spend the first consultation undoing that. Redirecting. Starting over.
Because male factor infertility contributes to nearly 50% of all cases, and it is still being treated as an afterthought in how couples approach their first fertility workup.
A semen analysis is a single, non-invasive test. It costs a fraction of what a hormone panel costs.
And it should be the first investigation done, not the last.
If you are a clinician reading this, make it a routine part of your initial recommendation. If you are a couple reading this, ask for it at your very first appointment.
