Emre Seli: Useful Information for Counseling Patients After an IVF Cycle with Complete Embryo Developmental Arrest

Emre Seli, Professor at Yale School of Medicine, shared a post on LinkedIn about a paper he co-authored with colleagues published in Fertility and Sterility:

“I’m pleased to share our recent publication in Fertility and Sterility:

‘Complete embryo developmental arrest and its prognostic significance in in vitro fertilization.’

One of the most difficult outcomes in IVF is complete embryo developmental arrest, when none of the fertilized embryos reach the blastocyst stage.

We asked a practical and important question: What does complete embryo arrest in one IVF cycle tell us about the outcome of a subsequent cycle?

In this propensity score-matched analysis, we found that patients with a history of complete embryo arrest had lower fertilization rates and substantially lower blastulation rates in their subsequent IVF cycle, with a higher likelihood of experiencing complete arrest again.

Importantly, however, euploidy rates among the blastocysts that were formed were not different.

We also examined whether changing the stimulation protocol or trigger type between cycles was associated with improved outcomes. In this analysis, protocol or trigger changes were not associated with improved blastulation or a reduced risk of complete arrest.

These findings provide useful information for counseling patients after an IVF cycle with complete embryo developmental arrest and raise important questions about the underlying biology of this phenomenon.
