“Hydrosalpinx: The Tube That Can Sabotage IVF

A hydrosalpinx is more than a blocked fallopian tube.

It is a fluid-filled, pathologically dilated tube that can negatively influence the uterine environment- even when the embryo is transferred directly into the uterus.

The proposed mechanisms include:

  • Toxic/inflammatory tubal fluid,
  • impaired endometrial receptivity,
  • altered embryo,
  • endometrial interaction,
  • reduced implantation potential.

Reflux of hydrosalpinx fluid into the uterine cavity may also mechanically disturb the endometrial environment.

This is why identifying a hydrosalpinx can be clinically important before embryo transfer, particularly in patients undergoing IVF.

Management is individualised, but evidence has supported addressing communicating hydrosalpinges before IVF in appropriate patients, with options including laparoscopic salpingectomy or proximal tubal occlusion depending on the clinical situation.

The key lesson:

In IVF, the fallopian tube may no longer be required for embryo transport – but its pathology can still matter.

Sometimes, improving IVF outcomes begins not in the embryology laboratory, but with recognising what is happening outside the uterus.”
