”Sperm Morphology, A Practical Laboratory Guide

Accurate sperm morphology assessment is essential in semen analysis. It requires standardized staining, strict morphological criteria, and experienced microscopic evaluation.

Key Morphological Defects

  • Head: Macrocephalic, microcephalic, pyriform, and amorphous heads; abnormal acrosomal features and vacuoles.
  • Neck and Midpiece: Bent neck, abnormal insertion, and irregular midpiece.
  • Tail: Short, bent, coiled, or multiple tails.
  • Cytoplasm: Excess residual cytoplasm.

Laboratory Pearl

According to the WHO Laboratory Manual for the Examination and Processing of Human Semen, 6th edition (2021), the lower fifth-percentile reference value for normal sperm morphology is approximately 4%, using strict criteria.

This is a reference value, not a definitive fertility or infertility cutoff. Morphology should be interpreted alongside sperm concentration, motility, total sperm number, and clinical findings.

Best Practice

  • Use validated staining methods and standardized assessment criteria.
  • Examine slides at approximately ×1000 magnification with oil immersion.
  • Maintain quality assurance and observer competency.
