Elisabet Stener-Victorin: Metabolic Dysfunction at the Core of PMOS

Elisabet Stener-Victorin, Associate Professor and Senior Lecturer in the Department of Physiology and Pharmacology at Karolinska Institutet, shared a post on LinkedIn about a paper she co-authored with colleagues published in Cell Metabolism:

“A new name, an old crisis.

Our Commentary is out in Cell Metabolism by Cell Press, with Han Zhao, Ricardo Azziz and Helena Teede.

Polycystic ovary syndrome, now polyendocrine metabolic ovarian syndrome (PMOS), was never a disease of the ovary.

Metabolic dysfunction sits at its core, and the evidence is not subtle:

  • Insulin resistance in 75–90% of patients, independent of adiposity
  • 5-fold increased risk of type 2 diabetes
  • Cardiovascular risk that persists in lean women (HR 1.40 at BMI <25)
  • 2-fold higher MASLD risk
  • Uterine cancer HR 5.76

And the research investment? NIH funding averaged $31.84 million per year from 2016 to 2022 – against $454 million for type 1 diabetes and $609 million for rheumatoid arthritis and lupus, conditions of comparable prevalence and life impact.

A condition affecting 8-13% of women worldwide remains undertreated, underfunded, and fragmented across specialties.

The skeletal muscle, adipose, hepatic, and inflammatory pillars of PMOS are among the least studied tissues in the disease.

That is where the answers are.

Renaming is a step change, not the resolution, the beginning of an overdue scientific reckoning with a disorder that has been hiding in plain sight.”
