The 4 Main IVF Stimulation Protocols – Fertility Plus

”The four main IVF stimulation protocols – explained in a line each.

Every IVF cycle follows the same path: baseline assessment, ovarian stimulation, monitoring scans, a trigger injection, then egg collection 34–36 hours later. What changes between protocols is how we stop your body ovulating too early.

Long GnRH agonist (luteal) – you start a daily agonist on day 21 of the previous cycle, then begin stimulation once your body is fully quiet. It’s the most established route: strong, predictable control, though it’s a longer cycle and uses more medication.

Short GnRH agonist (flare-up) – the agonist starts on day 2 and stimulation on day 3, using your body’s own initial surge of FSH and LH to help recruit follicles. Often chosen for women who are older or who’ve had a lower response before.

Ultra-short flare plus antagonist – three days of agonist to spark that early flare, then an antagonist added around day 5–6 to keep the cycle steady. A useful option where response has been limited previously.

GnRH antagonist – you begin stimulation on day 1 and add the antagonist later, either on a fixed day or once your leading follicle reaches about 14mm. No long down-regulation phase, simpler timing, and helpful flexibility where reducing the risk of OHSS matters.
