Yasmin Magdi: Critical Concerns In POSTAL Study on Thyroid and IVF

Effect of Levothyroxine on Miscarriage Among Women With Normal Thyroid Function and Thyroid Autoimmunity Undergoing In Vitro Fertilization and Embryo Transfer: A Randomized Clinical Trial. JAMA. 2017 Dec 12;318(22):2190-2198. doi: 10.1001/jama.2017.18249. PMID: 29234808’ has been published in Frontier of Endocrinology.

Why investigating this RCT is important:

  • Compromised integrity puts our patients at risk of ineffective or even harmful treatment.
  • This is the largest RCT available. A recent meta-analysis included 4 RCTs with total 787 patients (PMID: 30249251), 600 of them are from the POSTAL study.

Key findings:

  • Using the TRACT checklist, major concerns in five domains out of seven regarding retrospective trial registration, baseline imbalances, inappropriate data reporting, mismatch between data presented in ESHRE abstract and manuscript and timeline discrepancies were identified.
  • The overall integrity risk of this RCT is high

What’s next:

  • Journal, publisher and institutions have an ethical obligation to the community to go through investigations
  • A call for standardization of stringent governance and adherence to international reporting guidelines and recommendations
