Nitin Kataria: Rethinking Embryo Grading in Modern IVF Practice

“Embryo grading: beyond numbers, towards understanding potential

As embryologists, we often find ourselves translating microscopic observations into meaningful clinical decisions. Embryo grading is one such critical step—where science meets judgment. Traditionally, embryo grading has been based on morphological assessment:

  • Cleavage-stage embryos: symmetry, fragmentation, blastomere number
  • Blastocysts: expansion, inner cell mass (ICM), and trophectoderm (TE) quality

While these grading systems provide a standardized framework, it’s important to remember:

  • A ‘perfect-looking’ embryo is not always euploid
  • A ‘poor-grade’ embryo can still result in a healthy live birth

So what does grading truly represent?

It reflects ‘developmental competence at a given time point’, not absolute viability.

In modern IVF practice, embryo selection is evolving:

  • Time-lapse imaging offers dynamic developmental insights
  • Genetic testing (PGT-A) adds a chromosomal dimension
  • Clinical context – maternal age, history, endometrial receptivity – remains crucial

As senior embryologists, our role is not just to grade embryos, but to:

  • Interpret morphology in context
  • Communicate realistic expectations to clinicians and patients
  • Continuously adapt to emerging technologies

Embryo grading is not a verdict – it’s a guide.

And like all guides, it works best when combined with experience, data, and clinical judgment.”
