Can Pipelle-induced endometrial injury modify the uterine environment in women with recurrent implantation failure?

In this new episode, we discuss a study exploring how intentional endometrial injury may trigger local biological changes in patients with recurrent implantation failure — RIF.

The research shows that RIF patients initially presented with abnormal endometrial tissue structure, fewer pinopodes, and an imbalance in molecular markers related to receptivity. After biopsy-induced injury, the authors observed ultrastructural improvements and increased expression of leukemia inhibitory factor — LIF, bringing the endometrial profile closer to that seen in fertile controls.

However, other markers, including estrogen receptors and specific integrins, remained largely unchanged. This suggests that the biological response to injury is selective, rather than universal.

A valuable episode to critically discuss controlled endometrial trauma, local immune activation, tissue remodeling, and their potential relevance for embryo implantation.

The message is balanced: promising exploratory findings, but larger clinical trials are needed before drawing firm clinical conclusions.
