“We often blame egg quality for age-related fertility decline.

But the data tells a different story: the uterus ages too – and it plays an independent, significant role.

Here is what the latest reproductive science reveals about the aging endometrium:

Structural Changes:

  • Endometrial thickness decreases by an average of 0.52mm.
  • Progressive loss of spiral arteries leads to reduced blood flow.
  • Increased fibrosis (scarring) and atrophy alter the physical environment.

Molecular and Cellular Shifts:

  • Overexpression of aging genes (FOXC1) and loss of essential epigenetic marks (H3K27ac).
  • Increased cellular senescence and inflammatory ‘inflammaging’ (IL17RB).
  • Decline in progesterone receptors (PGR) and receptivity markers (HOXA10), making the lining less responsive to hormones.

The Clinical Impact (Hard Numbers):

Even when we control for egg quality using donor eggs:

  • Women 49+ had a 31.7% live birth rate vs. 46.2% in women 35–40.
  • Miscarriage rates jumped from 24.2% to 37.6%.
  • Overall, advanced age lowers implantation odds by 27% and clinical pregnancy by 20% – independent of the embryo.

The uterine factor is real. As we push the boundaries of reproductive medicine, we must invest in anti-aging strategies for the uterine environment, not just ovarian reserve.”
