“Ovarian Tissue Cryopreservation – Fertility Preservation Beyond Egg Freezing
Fertility preservation is no longer limited to cryopreserving mature oocytes.
Ovarian tissue cryopreservation (OTC) involves:
- laparoscopic removal of cortical ovarian tissue,
- cryopreservation of primordial follicles,
- later autotransplantation when fertility restoration is desired.
The biological advantage is significant: instead of preserving a limited number of mature oocytes, OTC preserves thousands of primordial follicles within their native stromal environment.
It can be particularly valuable when gonadotoxic treatment cannot be delayed, especially in children and young patients where conventional oocyte stimulation may be impractical.
Following transplantation, ovarian tissue can potentially restore endocrine function and fertility, with reported live births demonstrating that cryopreserved ovarian tissue can retain reproductive potential.
The field is also evolving toward in-vitro follicle maturation, artificial ovary systems, and improved tissue – engineering approaches, aiming to expand future fertility- preservation options.
From preserving individual oocytes to preserving the ovarian follicular reserve itself.”
