Anuradha Housnur, Academy and Division Head of Clinical Embryology at Lall Baby Steps, shared a post on LinkedIn:
” ‘ICSI Is Not About Injecting the Sperm. It Is About Reading the Oocyte.’
A technically correct ICSI can still fail if the embryologist does not understand what the oocyte is telling them.
Here are a few practical principles that distinguish precision ICSI from routine ICSI:
1. Don’t rush the oocyte.
Assess the oocyte carefully before injection. Maturity, cytoplasmic appearance, polar body position and the overall oocyte morphology can influence how you approach the injection.
2. Your pipette movement matters.
ICSI is not about how quickly you reach the ooplasm. It is about controlled movement, appropriate pressure and understanding the resistance offered by the oolemma.
3. Oolemma breakage should be deliberate.
A sudden or forceful rupture can create unnecessary trauma. Learn to recognise the subtle change in resistance rather than relying purely on speed.
4. Sperm immobilisation is not merely a checkbox.
The way the sperm is selected, immobilised and handled can influence the precision of the injection.
5. Don’t judge fertilisation too early.
Post-injection oocyte appearance is not the same as fertilisation assessment. Proper fertilisation assessment requires standardised timing and interpretation of pronuclear development.
6. The real skill is consistency.
One beautiful ICSI does not make an embryologist exceptional. Reproducibility, traceability, documentation and consistent outcomes do.
