”22 eggs. Eight blastocysts. One embryo available for transfer.
Mohini and Palash travelled from Italy for their second child. Their evaluation uncovered more than an age-related fertility challenge: a markedly elevated sperm DNA fragmentation result, metabolic and thyroid concerns, and an isthmocele in Mohini’s previous caesarean scar. They took time to prepare, and her scar was repaired in Italy before they returned for IVF.
At 39, a reassuring AMH can mean more eggs but it cannot guarantee chromosomally normal embryos. Of eight blastocysts tested in their cycle, only one was reported euploid. We transferred it in a carefully planned medicated cycle, and Mohini conceived on the first transfer.
The lesson isn’t that every couple needs PGT-A, every elevated DFI rules out natural pregnancy, or every scar niche requires surgery. It’s that secondary infertility deserves a thorough, individualised assessment of BOTH partners and the uterus.
One embryo. A healthy pregnancy. And a reminder that the work before IVF matters, too.
