Rana Mondal: Can We Replace Daily Fertility Injections with a Simple Daily Pill?
Rana Mondal, Senior Clinical Fellow in Obstetrics and Gynaecology with Special Interest in Early Pregnancy at Bedfordshire Hospitals NHS Foundation Trust, shared a post on LinkedIn about a paper he co-authored with Mouli Nandi, published in the International Journal of Gynecology and Obstetrics:
“Can we really replace daily fertility injections with a simple daily pill?
That is the exact question behind our newly published paper in the International Journal of Gynecology and Obstetrics (FIGO/Wiley).
While oral GnRH antagonists like Relugolix and Elagolix sound like a complete game-changer for patient comfort, our real-world clinical findings show there is a catch.
Here is what our analysis of 173 donor cycles revealed:
- Pituitary Protection Works: Both oral and injectable protocols showed reliably low premature LH surge rates (around 3.4%).
- The Maturity Gap: Injectable Cetrorelix delivered significantly higher mature oocyte rates (83.9% vs. ~75% with oral agents).
- The Efficiency Tax: Oral protocols demanded substantially higher total gonadotropin doses and dragged out stimulation cycles by up to 3 extra days.
- Embryo Impact: Cetrorelix cycles yielded more usable blastocysts, primarily because more mature eggs were collected in the first place.
- Zero OHSS: All groups maintained high safety thanks to GnRH agonist triggering.
The takeaway: Oral antagonists definitely make cycles easier on the donor, but convenience comes at the cost of cycle efficiency, drug dose, and mature oocyte yield.”
