A failed IVF cycle is not simply ՛try again՛.
It’s clinical data.
Here’s why that framing matters more than it sounds:
Cumulative live birth rates climb sharply with well-managed repeat attempts – from roughly 86% by the sixth cycle for patients under 35, down to about 42% for patients 40 and older.
The gap between those two numbers isn’t just age. It’s how much a clinic learns from each attempt before the next one.
Strategic reattempt protocols treat every unsuccessful cycle as a diagnostic checkpoint. Before deciding on the next step, that means reviewing:
- Ovarian response
- Embryo quality
- Endometrial receptivity
- Prior protocol fit
In other words: the value isn’t repeating the same approach and hoping for a different result.
It’s structured patient selection – knowing who benefits from a protocol adjustment, who needs more workup first, and who’s better served by a different pathway altogether.
Here’s the part that complicates the obvious instinct to ‘just change something’:
Switching stimulation protocols reflexively after a failed cycle doesn’t reliably improve lab outcomes. Some data even show repeating the same protocol produces comparable, sometimes slightly better, results than switching for the sake of switching.
The gain isn’t from changing the protocol. It’s from changing it for a specific, data-backed reason – or deliberately confirming that nothing needs to change at all.
Here’s what that actually changes:
Clinics applying this framework are seeing patients move through fewer, better-targeted cycles – each attempt informed by what the last one revealed, instead of starting from zero.
And that changes the question specialists should be asking.
Not ‘should we try again’.
