“The Trisomy Enigma: Why Do Some ‘Excellent’ Embryos Fail to Implant?
As embryologists, we often encounter a frustrating paradox: a morphologically ‘perfect’ Grade A blastocyst that fails to result in a successful pregnancy. While many factors are at play, the genetic integrity of the embryo remains the most critical determinant.
Why is Trisomy the ‘Silent Saboteur’ of IVF Success?
Trisomy – the presence of an extra chromosome – is the most common form of aneuploidy. It is a primary driver behind:
- Recurrent Implantation Failure (RIF): Where the embryo lacks the correct genetic ‘blueprint’ to establish a molecular dialogue with the endometrium.
- Early Pregnancy Loss: Nature’s selective mechanism to halt the development of chromosomally abnormal embryos.
Cutting-Edge Insights:
Recent research highlights that Oocyte Aging leads to the degradation of Cohesin proteins, which are essential for maintaining chromosome cohesion. This degradation often results in Non-disjunction during meiosis, leading to Trisomy.
The Bottom Line:
Morphological grading alone is no longer enough, especially in complex cases. Integrating expert embryological assessment with Preimplantation Genetic Testing (PGT-A) is key to improving clinical outcomes and reducing the time to a healthy live birth.
