“A C-section scar can heal on the outside, but what about inside?
A cesarean delivery may leave more than a visible scar.
For some women, a defect can develop in the uterine wall at the previous cesarean incision site.
This is often called a cesarean scar defect, isthmocele, or niche.
And sometimes, the symptoms appear months or even years later.
What Can It Cause?
- Postmenstrual spotting or prolonged bleeding after periods
- Pelvic pain or cramping
- Pain during sex
- Abnormal uterine bleeding
- Difficulties with conception in some women
- Concerns in future pregnancies, depending on the scar defect
But here’s the important part:
Not every cesarean scar defect causes symptoms.
Some women have a niche without significant problems, while others may experience persistent bleeding, pelvic symptoms, or fertility concerns.
How Is It Investigated?
Evaluation may include transvaginal ultrasound, saline-infused sonography, hysteroscopy, or MRI in selected cases.
Treatment depends on the symptoms, size and location of the defect, residual myometrial thickness, fertility plans, and future pregnancy considerations.
Options may range from observation or medical management to:
- hysteroscopic,
- laparoscopic,
- vaginal,
- other surgical approaches in appropriately selected patients.
The overlooked message:
A cesarean scar is not simply a mark on the abdomen.
For some women, it can become a long-term gynecologic issue involving bleeding, pain, fertility, and future pregnancy planning.
Understanding cesarean scar disorders can help move care from ‘the surgery is long over’ to ‘let’s understand what is happening now.’
