“Preventing OHSS in Women with PCOS/PMOS: Every Step Matters
Women with polycystic ovary syndrome (PCOS/PMOS) are at a significantly higher risk of developing ovarian hyperstimulation syndrome (OHSS) during IVF treatment. Fortunately, advances in stimulation protocols have made severe OHSS increasingly preventable.
Current preventive strategies may include:
- Cabergoline, which is generally preferred over bromocriptine because it has stronger evidence for reducing the severity of OHSS and is usually better tolerated.
- Letrozole in selected patients.
- A dual trigger in carefully chosen cases.
- A freeze-all strategy, delaying embryo transfer until a later cycle to avoid the effects of rising hCG levels.
Recently, we managed a patient with PCOS/PMOS who was considered to be at high risk for OHSS. Based on her response to stimulation, she could have progressed to severe OHSS. However, with careful monitoring and the use of preventive measures, her symptoms remained mild, and she recovered well.
This case is a reminder that successful IVF is not only about retrieving many oocytes – it is also about anticipating complications, individualizing treatment, and protecting patient safety.
As reproductive medicine continues to evolve, preventing OHSS should remain one of our highest priorities, especially in women with PCOS/PMOS.
