“Exploring the Complex Relationship Between SLE and APS — With Insights From the New 2025 Guidelines
Systemic lupus erythematosus (SLE) and antiphospholipid syndrome (APS) are often perceived as closely linked autoimmune conditions. While they share overlapping immunological features – such as autoantibodies and genetic susceptibility – current research confirms that they are distinct clinical entities within the systemic autoimmune spectrum.
Key insights:
Up to 40% of SLE patients test positive for antiphospholipid antibodies (APL).
A significant subset of primary APS patients carry ANA or dsDNA antibodies, yet do not develop SLE, even after long-term follow-up.
These nuances have become even more important with the release of the new 2025 SLE guidelines (ACR 2025, DGRh S3-Guideline 2025), which introduce major updates in:
- Diagnostic pathways and disease activity assessment
- Universal use of antimalarials unless contraindicated
- Stronger emphasis on glucocorticoid minimization, targeting ≤5 mg prednisone/day
- Earlier and more tailored use of immunosuppressants and biologics
- Structured monitoring strategies for long-term organ protection
- Clear recommendations for special situations, including pregnancy and APS overlap
