“ATA 2026 Guidelines: A New Era in Thyroid Care During Pregnancy

The American Thyroid Association (ATA) 2026 Guidelines introduce important updates in the management of thyroid disease during preconception, pregnancy, and postpartum care.

  • The traditional universal TSH 2.5 mIU/L first-trimester cutoff is no longer recommended
  • Greater emphasis on trimester-specific, population-specific, and laboratory-specific reference ranges
  • TSH >4.0 mIU/L is generally considered abnormal when pregnancy-specific ranges are unavailable
  • All overt hypothyroidism should be treated with levothyroxine
  • Women already receiving levothyroxine should increase the dose by approximately 20–30% once pregnancy is confirmed
  • Routine levothyroxine is not recommended for euthyroid, TPO-antibody–positive women with infertility
  • New focus on avoiding overtreatment, individualized care, and shared decision-making
  • Updated guidance on Graves disease, antithyroid drug selection, postpartum thyroiditis, and postpartum dose adjustment
