Khalil Faaed: We Can Grade an Embryo, but Can We Really Predict Its Future?

Under the microscope, an embryo can look beautiful:

Its cells may appear well organized.
Its blastocyst may expand nicely.
Its inner cell mass and trophectoderm may receive an excellent grade.

And naturally, we may think: This must be the embryo with the best chance.

But biology is rarely that simple.

Embryo grading tells us what we can see. It does not necessarily tell us everything that is happening inside the embryo.

An embryo that looks excellent may not implant. And an embryo with a less impressive appearance can sometimes lead to a healthy pregnancy.

This is one of the most important challenges in modern embryo selection.

We are moving beyond traditional morphology toward time-lapse imaging, morphokinetics, AI-assisted assessment, genetic information, and other emerging biological approaches.

The goal is not simply to find the embryo that looks the best. The goal is to understand which embryo has the strongest evidence of reproductive potential.

But we should also remember something important: More data does not automatically mean better prediction.

AI can identify patterns.
Time-lapse can reveal development over time.
Genetic testing can provide additional information.

But none of these approaches, alone, can tell the complete story of an embryo.

Perhaps the future of embryo selection is not about finding a single ‘perfect’ test.

Perhaps it is about bringing different pieces of biological information together and giving embryologists a clearer picture than morphology alone can provide.

Because when we look through the microscope, we see an embryo.

The real challenge is understanding everything we cannot see.”
