“Is a normal BMI enough to know you’re metabolically healthy before trying to conceive?

Increasingly, no.

India’s 2025 obesity guidelines moved past BMI alone, adding waist circumference and waist-to-height ratio to catch abdominal fat risk that weight alone misses.

Waist-to-hip ratio is a related, older WHO measure worth knowing too, but it isn’t the metric this year’s Indian guideline update centres on – worth being precise about, since the two get conflated online.

Why does this matter in my clinic specifically?

Central fat tracks closely with insulin resistance, and insulin resistance is already one of the tests I recommend for nearly every couple, given how common PCOS is in Indian women.

A lot of my patients with PCOS carry central fat at a completely normal BMI – exactly the group this shift is meant to catch.”
