William Akakpo: Improving Varicocele Surgery Through Doppler Assisted Microsurgery

William Akakpo, Urologist-Andrologist at Ambroise Paré-Hartmann Private Hospital Group, shared a post on LinkedIn:

“Varicocele is the leading medical cause of male infertility.

Today, two reference treatments are offered: microsurgery performed by the urological surgeon and embolization performed by the interventional radiologist.

Several years ago, I had the opportunity to develop and standardize the microsurgical approach with an initial concern to identify and not damage the testicular artery.

The risk of intraoperative arterial injury remains low (approximately 1%) but should be taken into consideration due to the risk of postoperative testicular atrophy.

The enlargement of the surgical field facilitates the identification of the testicular artery, the dissection of the veins to be ligated and the preservation of lymphatic vessels. However, our techniques must continue to evolve in order to improve patient care.

With this in mind, I evaluated the interest of the mini Doppler Vascular Technology in addition to microsurgical exploration. This sterile instrument, adapted to the size of the spermatic cord, participates in arterial identification without lengthening the operating time.

A larger-scale evaluation will make it possible to better define its place in the surgical strategy.
