Peter Humaidan: APHRODITE Criteria to Move Towards a Stratified Gonadotropin Stimulation in Infertile Males

Peter Humaidan, Clinical Director at pMorph Ltd, shared a post on LinkedIn about a paper published in Human Reproduction:

“Treatment of male infertility has until now mostly relied on the empirical use of oral agents to boost spermatogenesis, a fragmented approach resulting in a lack of cohesive strategy, and a high proportion of untreated males. The oral treatment usually results in suboptimal results – the reason why oral treatment for ovarian stimulation in standard IVF was abandoned years ago in women.

Since ICSI was introduced, the interest in boosting spermatogenesis has been very limited in the belief that ICSI solves everything – and we are treating women for their partners problems, which is quite unique in medicine. Time has come to challenge the old dogma and see the infertile male as a patient, rather than a semen sample, and explore his life style, medication, anatomy, and possible exposure to environmental toxicants.

In this context Sandro Esteves and I, very recently published an Opinion paper in HR, using the new APHRODITE criteria to move towards a stratified gonadotropin stimulation in infertile males fulfilling the suggested criteria. Clearly we now need the prospective trials in well defined patient phenotypes, showing the efficacy of gonadotropin stimulation.

Once again- the infertile male is more than a semen sample – he is a patient, deserving the full attention of the IVF doctor, and we call for ‘Male fertility awareness’ as the new ‘mantra’.”
