Inshirah Sgayer: Happy to Share Our New Publication on Unstable Fetal Lie

‘Obstetrical and Neonatal Outcomes of Term Pregnancies Complicated by Unstable Fetal Lie’

Unstable fetal lie at term was uncommon in our cohort, occurring in 0.25% of deliveries, and evidence regarding its optimal management remains limited.

Among women who attempted vaginal delivery after stabilization to vertex, 69.0% delivered vaginally. However, compared with spontaneous vertex presentation, unstable fetal lie was associated with a higher intrapartum cesarean delivery rate (31.0% vs. 10.3%), including cesareans due to fetal distress, labor dystocia, and return to non-vertex presentation.

Importantly, neonatal outcomes were reassuring, with no cord prolapse, birth trauma, or perinatal death reported.

These findings support individualized delivery planning, careful intrapartum assessment, and repeated confirmation of fetal presentation during labor.

Grateful to all co-authors for their collaboration on this work.
