Sean Lauber: Global PGT-A Guidelines Show Ongoing Debate Over IVF Testing Use
Sean Lauber, Founder of Remembryo, shared a post on LinkedIn about a paper by Laura Sichenze et al. published in Reproductive Biology and Endocrinology:
“PGT-A is widely used in IVF, but experts don’t always agree on when it should be used or how the results should be handled.
Sichenze et al. (2026) compared 14 guidelines and position statements from 11 major IVF and genetics organizations around the world.
While most groups agreed that PGT-A may be useful for older patients, there was much less agreement about its use for recurrent implantation failure, recurrent pregnancy loss, severe male factor infertility, mosaic embryos, and prenatal testing after PGT-A.
Some suggested that PGT-A may be considered starting around age 35, while others focused on patients closer to age 38 or older, and some felt there still wasn’t enough evidence to recommend it routinely based on age alone.
There were also differences in recommendations for mosaic embryos. Most organizations agreed that mosaic embryos can be transferred when no euploid embryos are available, but they differed on how these embryos should be prioritized (by mosaic % level, specific chromosomes, whole level/segmental, etc.)
The authors suggest that these differences exist because PGT-A technology is changing quickly and there are still relatively few large, high-quality studies available. They recommend updating guidelines every 2 years so recommendations can keep pace with new evidence and technology.
