“Multiple Polar Bodies in a Human Oocyte: What Could They Indicate?
During routine oocyte assessment, multiple polar body-like structures may occasionally be observed within the perivitelline space.
A mature metaphase-II oocyte normally contains one first polar body before ICSI. Following normal fertilization and completion of meiosis II, a second polar body is extruded.
When more than the expected number is observed, possible explanations include:
- Fragmentation of the first polar body
- Oocyte ageing or prolonged culture
- Abnormal meiotic spindle activity
- Irregular chromosome segregation
- Premature or abnormal oocyte activation
It is important not to interpret multiple polar bodies as direct evidence of polyspermy. Fertilization must be evaluated separately by pronuclear assessment – normally 2PN – together with the number and appearance of the polar bodies.
Although abnormal or fragmented polar-body morphology may reflect altered oocyte competence, morphology alone cannot confirm aneuploidy. Continued embryo-development assessment and, when clinically indicated, genetic testing provide more reliable information.
Every unusual oocyte morphology reminds us how complex and precisely regulated human meiosis is.”
