Justin Houman: A Note for Primary Care and Sexual Medicine Colleagues on Peyronie’s Referrals
Justin Houman, Men’s Health Urologist at Tower Urology at Cedars-Sinai Medical Center, shared on LinkedIn:
“A note for primary care and sexual medicine colleagues on Peyronie’s referrals.
Prevalence estimates put symptoms consistent with Peyronie’s at roughly 11 percent of men, against under 1 percent carrying a formal diagnosis. Most of that gap is men who never raise it.
Three points that make the referral productive:
- Phase determines everything. Acute disease is the narrow window: confirm the diagnosis, manage pain, and track the curve to stability. The intralesional options are stable-phase therapies, and collagenase specifically is labeled for a palpable plaque with at least 30 degrees of curvature once stabilized.
- The evidence tiers are not equal. Collagenase is the only FDA-approved medication. Traction has modest monotherapy data with meaningful combination results. Verapamil is off-label with weak evidence, PRP remains experimental with a randomized comparison showing no curvature benefit, and shockwave is for pain rather than deformity.
- Not every man needs treatment. A stable, non-painful curve that does not impair intercourse can be observed, and saying so plainly is often what keeps a patient engaged.
For men with Peyronie’s and erectile dysfunction refractory to pharmacotherapy, prosthesis with intraoperative modeling addresses both in one operation.
Happy to see these patients from across the US, including by telemedicine.”
