“Empty Follicles After Trigger: How Common Is It Really?

Empty follicle syndrome (EFS) is one of the most frustrating challenges in IVF—when no eggs are retrieved despite normal follicular development. Historically, EFS occurred in about 2–7% of cycles after hCG trigger. However, with modern antagonist protocols using GnRH agonist triggers, rates are now lower—around 1–3.5%, and as low as 1–2% in hyper-responders.

Most cases stem from timing or administration issues rather than true ovarian failure, and prompt rescue with hCG can often salvage the cycle.
